of Health has introduced new and updated medical assistance in dying forms and
accompanying instruction documents.

B.C.’s website here.
to practitioners, assessors and coordinators, is the revision of the patient
request record (form 1632) which has moved the patient confirmation record
(final consent before administration) to a separate new form (form 1637). As
well, new instruction documents have been developed to help support patients
and providers fill in the medical assistance in dying provincial forms.
New pharmacy protocol and medication administration records,
introducing secobarbital as a second oral option, is also now available for
providers and pharmacists. Nurse
practitioners may contact their health authority’s Care Coordination
Centre to obtain a copy.
